​Scope of Practice
Sun City Massage’s therapists and instructors are licensed professionals who are held to the highest standards of the American Massage Therapy Association, the Texas Department of Licensing and Regulation, and the Texas Education Agency. Our therapists and/or instructors do not diagnose illness or disease or prescribe pharmaceuticals. None of the services provided are a substitute for medical attention.
In our commitment to providing a unique and outstanding experience to all of our clients and out of consideration for our therapists’ and clients’ time, we have adopted the following policies.
Conduct
All forms of misconduct are forbidden. Clients understand that any illicit or suggestive remarks, advances, or gestures will result in the immediate termination of the individual employee session, and a report will be filed with the company. The company will be responsible for any disciplinary action. Companies are under a “three strikes rule” and any documented incidents over three with the same company will result in the cancellation of the contract and forfeiture of future services and any prepaid funds.
Tardiness
Clients need to have a designated area ready 10 minutes prior to the scheduled appointment to allow the Sun City Massage & Wellness time to set up equipment. Any delay in time will be deducted from the service time.
In the event the Sun City Massage & Wellness arrives late to the agreed location, the client will either receive a refund for the time missed or have the equivalent time added to the original end time to make up the difference. The option used will be dictated by client desires and staff availability.
Cancellations
Appointments may be cancelled more than 24 hours. 
Payments
Payments are due at time of service unless otherwise expressed in writing.
Failure to make timely payments will result in a $35 per month late fee. Clients who do not submit payment for 90 days after the due date, will have their contract cancelled and
 will be barred from receiving future services until their account has been paid in full. Clients who do not submit payment for 180 days after the due date, will be referred to a collection agency.
Refunds
All refunds are subject to the cancellation policy. If there is an issue concerning the quality/ quantity of service, the client may receive a credit for the agreed upon amount or a refund will be issued to the method of payment. The final decision will be made by Sun City Massage & Wellness.
Returned Payment
If a payment is returned for insufficient funds, the client will be charged and insufficient funds fee of $45.00, as well as the full amount for services rendered. If a payment is returned twice, all future payments from the client must be prepaid 2 weeks in advance.
Discounts
Any discount due to a client must be presented before the service is complete. No retroactive discounts will be applied.
Appointment Reminders
Sun City Massage & Wellness may send appointment reminders, which could be in the form of a message left over the phone either with an individual, on an answering machine, a written reminder sent through the mail, or a text message to the phone number on file. No personal information will be disclosed in this message however time of appointment and type of massage may be revealed.
Health Care Disclosure
We may disclose health information for issues related to public health and health oversight; to notify or assist in notifying a family member; to seek payment from insurance; in any instance prescribed by law; and as otherwise outlined in the Sun City Massage & Wellness Notice of Privacy Policy.

This notice describes how your medical information may be used and disclosed by Sun City Massage & Wellness and how you can get access to the information.

Primary Treatment: Your health information may be used or disclosed to other health care professionals for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment.
Follow Up Treatment: Your health information may also be used by another therapist or our staff to send you information on the treatment and management of your medical condition. We may also send you a newsletter of information describing other health related products and service that we believe may interest you.
Health Care Operations: Your health information may be used to support day-to-day activities and management of the practice as necessary.
Law Enforcement: We may disclose your health care information to a law enforcement official for purposes such as identifying or locating a suspect or missing person, complying with a court order or subpoena, and reporting neglect, abuse, and domestic violence. It may also be necessary to contact the appropriate authorities in order to prevent a serious and imminent threat to the health or safety of a particular person or to the general public.
Public Health: We may disclose your health information to public health authorities for the purposes related to: preventing, controlling and monitoring disease exposure; injury or disability; and reporting problems with products and reactions to medications.
Public Health Oversight: We may disclose your health information so that they can monitor, investigate, inspect, or license people working in the health care industry or for government benefits programs.
Emergencies: We may disclose your health information to notify or assist in notifying a family member or another person responsible for your care about your medical condition or in the event of an emergency or your death.
Appointment Reminders: Your health information may be used by therapist or staff to send you appointment reminders, which may be in the form of a message left over the phone either with an individual, on an answering machine, or a written reminder sent through the mail.
Payment: Your health information may be used to seek payment from your health plan, from other sources, such as an automobile insurer, or from credit card companies that you may use to pay for services.
Individual Rights: You have certain rights under the federal privacy standards. These include the right to:
  • Inspect and copy your health information
  • Have your medical data stored in a secure location with restricted access
  • Request restriction on the use and disclosure of your protected health information
  • Submit corrections to your protected health information
  • Receive confidential communications concerning your medical condition and treatment
  • Receive an accounting of how and to whom your protected health information has been disclosed.
