Scope of Practice An Amazing Massage's therapists are licensed professionals who are held to the highest standards of the American Massage Therapy Association, and the Texas Department of Licensing and Regulation. Our therapists do not diagnose illness or disease or prescribe pharmaceuticals. None of the services provided are a substitute for medical attention. In our commitment to providing a unique and outstanding experience to all of our clients and out of consideration for our therapists’ and clients’ time, we have adopted the following policies. Conduct All forms of misconduct are forbidden. Clients understand that any illicit or suggestive remarks, advances, or gestures will result in the immediate termination of and full payment for the individual session. Clients will also be barred from returning. Tardiness Clients can be up to 10 minutes late and have their appointment honored. Any further delay on the client's part will be deducted from the service time. In the event the An Amazing Massage therapist is late or starts a session after the agreed upon time, the client will receive the full treatment, or the appointment can be rescheduled. The option used will be dictated by client desires and staff availability. Cancellations/No Show Appointments may be cancelled or rescheduled online up to 6 hours in advance. Clients must call to cancel or reschedule if it is within 6 hours of their appointment time. Cancelling less than 6 hours before an appointment or failing to cancel at all and not showing up (no show) will incur a Late Cancellation/No Show fee of $40. Payments Payments are due at time of service unless otherwise expressed in writing. Refunds If there is an issue concerning the quality/ quantity of service, the client may receive a credit for the agreed upon amount or a refund will be issued to the method of payment. The final decision will be made by An Amazing Massage. If services are discontinued cancelled by An Amazing Massage due to misconduct on the client’s part, the client will not receive a refund or credit of any kind. Returned Payment If a payment is returned for insufficient funds, the client will be charged and insufficient funds fee of $50.00, as well as the full amount for services rendered. If a payment is returned twice, all future payments from the client must be prepaid in cash. Discounts Any discount due to a client must be presented beforethe service is complete. No retroactive discounts will be applied. Appointment Reminders An Amazing Massage may send appointment reminders, which could be in the form of a message left over the phone either with an individual, on an answering machine, a written reminder sent through the mail, or a text message to the phone number on file. No personal information will be disclosed in this message however time of appointment and type of massage may be revealed. Health Care Disclosure We may disclose health information for issues related to public health and health oversight; to notify or assist in notifying a family member; to seek payment from insurance; in any instance prescribed by law; and as otherwise outlined in the An Amazing Massage Notice of Privacy Policy.

We may also send you a newsletter of information describing other health related products and service that we believe may interest you. Health Care Operations: Your health information may be used to support day-to-day activities and management of the practice as necessary. Law Enforcement: We may disclose your health care information to a law enforcement official for purposes such as identifying or locating a suspect or missing person, complying with a court order or subpoena, and reporting neglect, abuse, and domestic violence. It may also be necessary to contact the appropriate authorities in order to prevent a serious and imminent threat to the health or safety of a particular person or to the general public. Public Health: We may disclose your health information to public health authorities for the purposes related to: preventing, controlling and monitoring disease exposure; injury or disability; and reporting problems with products and reactions to medications. Public Health Oversight: We may disclose your health information so that they can monitor, investigate, inspect, or license people working in the health care industry or for government benefits programs. Emergencies: We may disclose your health information to notify or assist in notifying a family member or another person responsible for your care about your medical condition or in the event of an emergency or your death. Appointment Reminders: Your health information may be used by therapist or staff to send you appointment reminders, which may be in the form of a message left over the phone either with an individual, on an answering machine, or a written reminder sent through the mail. Payment: Your health information may be used to seek payment from your health plan, from other sources, such as an automobile insurer, or from credit card companies that you may use to pay for services.
